All of the information provided in this form is considered confidential and is not shared with any person or agency outside of Camp Good Days and Special Times, Inc.

Camp Good Days and Special Times require a medical form to be signed and completed in order to attend a camping program. Upon receipt of this application a link to the medical forms will be sent via email provided. Only then will the registration process be complete.

All Camp Good Days medical forms will be completed online.

Only COMPLETE registration forms will be processed. The COMPLETE form, once accepted, holds your spot at camp.

The programs at Camp Good Days are free of charge; however, a program can cost Camp Good Days, on average, $650 per camper, per week. If you have the resources, we would appreciate any help in defraying our costs.
